Request Accommodations

Students seeking accommodations or services such as academic support, housing,
meal plans, or temporary assistance should complete the following steps:

Step 1: Review Documentation Requirements

Before submitting a request, please review our Disability Documentation Criteria to
understand the type of information needed to support your accommodation request.

As part of this process, download and have your licensed healthcare provider complete
the Certification of Health Care Provider Form. This form is required to verify the
nature and impact of your disability or condition.

Step 2: Complete the Online Accommodation Request Form

Fill out the Accommodation Request Form. When completing the form, you will be asked to upload:

  • the completed Certification of Health Care Provider Form and
  • any additional documentation supporting your request.

Supporting documentation may include:

  • Educational or psychological evaluations
  • IEPs (Individualized Education Plans) or 504 Plans from prior educational
    settings
  • Letters of Accommodation from previous colleges or universities (for transfer
    or graduate students).

Please note: All documentation must be within the last five years of the current
academic year.

Step 3: Intake Meeting

After we receive your completed application and documentation, a member of our team
may request a meeting with you to discuss your accommodations if needed. This
meeting provides an opportunity to:

  • Review your submitted materials;
  • Discuss your needs and experiences;
  • Determine reasonable and appropriate accommodations.

Step 4: Submit Requests Early

To help ensure your accommodations are in place at the start of the semester, we
recommend the following timelines:

Academic Accommodations (New and Semester Requests)

  • Fall Registration: August 1st
  • Winter Registration: October 1st
  • Spring Registration: December 1st
  • Summer Registration: May 1st

Housing Accommodations

  • Returning Students: May 1st
  • New Students: July 15th

Late submissions will still be reviewed, but timely requests ensure smoother
coordination and implementation.
